1) For me the word "restaurant" means is a public place where you can not only order and eat delicious food, but to take rest, to listen to the music and to enjoy service. 2) Fast food is really widespread nowadays as it is rather tasty (much tastier then healthy food), you don't feel hunger after eating it, it is very cheap and when you come to buy fast food, you are served really quickly, so you don't have to wait while it is cooked. 3) The 'family' style restaurant, first of all, should serve food not only for adults, but offer special meals for children, or at least various kinds of ice-cream. Also there can be some shows or music. And of course smoking should be prohibited there! 4) The main difference between 'family style restaurant' and 'theme' restaurant is that the themes can be quit various, and not always suitable for children for instance.
